Our Commitment to Your Privacy
We are committed to protecting the privacy and security of your health information. Federal law, including HIPAA, requires us to safeguard your Protected Health Information (PHI) and to provide this Notice explaining how your information may be used and shared, your rights, and our responsibilities. We will notify you if a breach occurs that may compromise your information.
What Is Protected Health Information (PHI)?
PHI includes any information that identifies you and relates to your past, present, or future health care, condition, or treatment. This includes medical records and identifying information such as your address or other personal identifiers.
Your Rights and How We May Use Your Health Information
- You may ask to see or get a copy of your records, request a correction, and ask us to contact you confidentially (for example, at a different phone number or address). You may also request limits on certain uses/disclosures (and in some situations we must honor your request, such as when you pay outofpocket in full), request an accounting of certain disclosures, and get a paper copy of this Notice at any time. You can name a personal representative to act for you, and you may file a complaint without retaliation if you believe your privacy rights have been violated.
- We may use or share your information as permitted by law for treatment, payment, and health care operations, and for certain public health and safety activities, legal requirements (including law enforcement or court orders when required), and with business associates who support our operations under confidentiality obligations.
Uses and Disclosures That Require Your Written Authorization
We will only use or share your health information with your written permission for certain purposes, such as marketing, the sale of health information, or the disclosure of psychotherapy notes (when applicable). You may revoke your authorization at any time in writing.
Electronic Communications
If you choose to receive communications by email, text, or similar electronic means, you acknowledge that some messages may be unencrypted and carry certain risks. You may opt out of electronic communications at any time.
Special Protections for Substance Use Disorder (SUD) Records
Certain substance use disorder treatment records are protected by additional federal confidentiality rules (42 C.F.R. Part 2). When these rules apply, we will not disclose SUD information unless permitted by law, with your consent, or pursuant to a qualifying court order. We protect these records in accordance with both HIPAA and Part 2 requirements.
Our Responsibilities
We are required by law to maintain the privacy and security of your PHI, follow the duties and privacy practices described in this Notice, and provide you with a copy. We will notify you promptly if a breach occurs that may have compromised the privacy or security of your information, and we will not retaliate against you for filing a complaint. We may change the terms of this Notice, and any changes will apply to all information we maintain; the updated Notice will be available upon request and posted on our website.
