Coordinated Care

At ARcare, we provide coordinated care to ensure that patients in central and northeastern Arkansas have all the help they need. Under our coordinated care services, you will be following a comprehensive treatment plan that covers your physical, mental and emotional well-being while also helping you manage your condition.

Parts of a coordinated care plan might involve some or all of the following services:

  • Care from a primary doctor
  • Inpatient care
  • Care from a specialist
  • Long-term care
  • Mental health services, such as counseling
  • Medical history
  • Lab work and test results
  • Home care
  • Caregivers that are family or formal
  • Education, resources and support for family members of patients
  • Prescription and nonprescription medications
  • Community resources

Our providers will determine which services you need for your coordinated care plan based on several factors, including the severity of your condition, your usual day-to-day activities and the type or types of medical conditions you have. Patients who come to ARcare for coordinate care can rely on us to help them come up with a plan that provides them with the right types of care.
